Requirements

• Current, active FloridaLPN license
• Current BLS CPR card (obtained in-person, not online)
• TB Skin Test (PPD) or TB Blood Test (QF)
• Alzheimers training - 2 Hour DOEA Approved Course (provided at no cost if needed)
• Hour DOEA Alzheimers Video (provided free of cost on DOEA website)
