Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, aligning with department objectives.
- Serve as co-leader in an interdisciplinary approach to patient groups, collaborating with other care professionals.
- Perform comprehensive evaluations, establish individualized treatment plans, and implement therapy interventions using established protocols.
- Develop and communicate individualized discharge plans; ensure documentation is accurate and timely.
- Supervise and delegate tasks to assigned assistants, and maintain thorough clinical documentation within required timelines.
- Participate in ongoing care coordination and patient progress communication with the care team and families.

Requirements
- Education: Doctor of Physical Therapy (DPT) degree.
- Licenses/Certifications: Physical Therapy licensure in the state of practice (CA) required; CPR/BLS certification from an approved provider required.
- Experience: At least two years of recent experience in the field.
- Credentials: Valid driver’s license (age 21 or older) required.
- Additional: EPIC experience; current license to practice physical therapy in applicable states of practice.
