Physical Therapist

Physical Therapist Home Health | Inpatient | Outpatient Rehab Key Responsibilities:
- Deliver skilled physical therapy services in inpatient, outpatient, and home health settings
- Create and adjust individualized care plans to meet functional and mobility goals
- Educate patients and families on exercise programs, safety, and rehabilitation strategies
- Document evaluations, treatments, and progress according to regulatory and organizational standards
- Collaborate closely with nurses, physicians, and other therapists to ensure coordinated, high-quality care
- Participate in case conferences and discharge planning
- Help supervise PTAs and assist with training and mentoring if needed
- Degree in Physical Therapy from an accredited program
- Active South Dakota PT license (or ability to obtain)
- Valid driver's license and reliable transportation for home visits
- Current BLS certification
- Prior experience in any of the following preferred: home health, outpatient ortho, or acute rehab
- Strong interpersonal and time-management skills
