Physical Therapist

Home Health | Inpatient | Outpatient Rehab

Key Responsibilities:

Deliver skilled physical therapy services in inpatient, outpatient, and home health settings
Create and adjust individualized care plans to meet functional and mobility goals
Educate patients and families on exercise programs, safety, and rehabilitation strategies
Document evaluations, treatments, and progress according to regulatory and organizational standards
Collaborate closely with nurses, physicians, and other therapists to ensure coordinated, high-quality care
Participate in case conferences and discharge planning
Help supervise PTAs and assist with training and mentoring if needed

Qualifications:

Degree in Physical Therapy from an accredited program
Active South Dakota PT license (or ability to obtain)
Valid driver's license and reliable transportation for home visits
Current BLS certification
Prior experience in any of the following preferred: home health, outpatient ortho, or acute rehab
Strong interpersonal and time-management skills
