Care Coordinator

Role: Care Coordinator for the Birth to Three Program, supporting children aged 0-3 with developmental delays or disabilities and their families. Responsibilities: Partner with families, service providers, and community resources to support child development. Conduct functional assessments and develop individualized, family-centered service plans. Coordinate services, facilitate team meetings, and support families through education and advocacy. Maintain accurate documentation, meet productivity expectations, and adapt schedules to client needs, including occasional evenings or weekends. After training, enjoy remote work flexibility with mandatory in-office meetings biweekly. Qualifications: Bachelor's degree in Social Work, Child Development, or related fields. Preferably, one year of supervised experience with families with special needs. Valid driver's license, reliable transportation, and strong communication, organizational, and documentation skills are essential. Ability to work in diverse settings and respond appropriately to crises is required. Work Environment: Community-based with daily travel, moderate physical activity, and exposure to various environments, including home and community settings. May include occasional overnight stays.
