Registered Nurse – Home Health

Schedule: 8hrs

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• CHPN certification (preferred)
• Proof of current CPR certification
• Current driver's license
• Must have reliable transportation and insurance
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience

Roles & Responsibilities:

• Assess patients in home health and/or hospice settings to identify their physical, psychosocial, and environmental needs.
• Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and their families about care routines, diet, medications, and home safety tailored to their specific needs.
• Supervise and provide clinical direction to Home Health Aides and assistive nursing staff to ensure quality service delivery.
• Perform pain assessments and utilize appropriate pain management techniques.
• Communicate effectively with interdisciplinary teams, patients, and families.
