Key Responsibilities\n

• Conduct thorough psychosocial assessments to inform individualized treatment plans, incorporating evidence-based social work interventions tailored to meet patient needs.

• Collaborate closely with the healthcare team, including physicians, nurses, and mental health professionals, to develop and refine discharge plans that incorporate the patient’s wellness goals.

• Provide crisis intervention and support, effectively assessing and addressing emotional and behavioral issues that patients may face during their recovery journey.

• Maintain accurate, clear, and timely clinical documentation, including progress notes and treatment records, adhering to departmental policies and best practices.

• Engage with family members and support systems to facilitate understanding and involvement in the patient's treatment and discharge planning process, ensuring a supportive environment for recovery.

Patient Population / Client Focus\n

In this role, you will primarily serve individuals undergoing detoxification and recovery from substance use disorders. Many patients may present with a range of psychological conditions, including anxiety, depression, and trauma-related issues that require sensitive and informed intervention strategies. Qualifications\n

• A Master's degree in Social Work or Counseling from an accredited institution is required.
