Remote RN Care Manager: LTSS & Care Coordination
Molina Healthcare of Illinois
Molina Healthcare in Wisconsin is seeking an RN care manager to support LTSS coordination and care management for high-need members. You will collaborate with a multidisciplinary team to ensure delivery of quality, cost-effective care and optimal outcomes.
The role requires comprehensive assessments, waiver activities, care planning, and travel within Barron County. Strong communication and motivational interviewing are essential to success in this position.
