Physical Therapist - Travel Contract

Travel Physical Therapist – Outpatient – Niceville, FL

Provide outpatient physical therapy services evaluating and treating patients to restore function and promote return to community activities. Perform comprehensive evaluations, develop and implement individualized treatment plans, document care accurately, and collaborate with physicians, patients, families, and multidisciplinary team members. Maintain professional conduct, uphold patient and employee safety practices, and participate in performance improvement and student education activities.

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Active Florida Physical Therapy license required to start the assignment.
• BLS Certification from AHA or ARC may be required.
• Experience in outpatient physical therapy providing evaluations and 60-minute initial evaluations with treatment.
• Ability to complete timely, accurate medical record documentation including goal writing, functional quality indicators, letters of medical necessity, and charge entry.
• Knowledge of state practice act and regulatory requirements. Maintain required continuing education and licensure in good standing.
• Demonstrated competence in safe patient handling and infection control practices.

Preferred Qualifications

• Experience in an outpatient clinic environment with delegation and direction of discipline-specific therapy services.
• Prior experience participating in student education or precepting.
• Engagement in professional development activities and familiarity with performance improvement processes.

Assignment Details

• Shift pattern: 5x8.
• Location: Outpatient clinic in Niceville, FL.
• May be required to float to other local facilities as needed.
