General Summary Postgraduate Year One (PGY1) pharmacist resident is a pharmacist in training who provides comprehensive pharmaceutical care under the supervision of preceptors. The resident will provide a level of service consistent with expectations of a clinical pharmacist and patient care pharmacist. Responsibilities Medication/Solution Order Review – Review prescriber's orders for appropriateness relative to the patient's condition, height, weight, and age. Review compliance with legal requirements, institutional policies and procedures, and drug use restrictions in accordance with prescribing limits set forth by the Pharmacy and Therapeutics Committee and the Medical Executive Committee. Medication/Solution Order Processing – Utilize the full functionality of the automated order entry system to facilitate accurate and timely medication/solution distribution and administration. Conduct allergy review, duplicate therapy check, drug-drug interaction screening, dosage range validation, IV compatibility and food-drug interaction screening. Document interventions using approved documentation tools. Pharmacy Information System Competency required. Technician and Ancillary Staff Supervision – Review medications prepared for distribution by technicians for accuracy, appropriateness, and timeliness. Monitor technician accountability to follow established policies and procedures for medication delivery, including the automated distribution system. Review technician preparation of intravenous admixtures, parenteral nutrition compounding, chemotherapy and miscellaneous sterile products. Clinical Responsibilities – Maintain daily clinical logs focusing on continuity of care for patients assigned to the Pharmacy Consult services for Parenteral Nutrition, Pharmacokinetics, warfarin, etc. Provide patient education about drug therapy. Interpret laboratory data in context with medication use. Determine alternative therapies to achieve outcomes. Drug Information Resource – Provide pertinent drug information to medical and ancillary hospital staff. Research medication dosages, indications, interactions, compatibility, and administration when requested by healthcare professionals. Author pharmacy newsletter articles and conduct staff development and educational services to pharmacy staff, healthcare professionals and the community through outreach programs. Conduct formal Continuing Education presentations to the Department of Pharmacy. Education Resource – Conduct staff development programs and provide educational services to pharmacy staff, pharmacy students and residents; nursing staff, physicians, and other healthcare professionals. Answer drug information questions upon request by pharmacists and other health care professionals. Departmental Performance Improvement Participation – Participate in departmental performance improvement by ensuring that all quality control measures are followed and properly documented. Monitor Automated Distribution Cabinet Report Reviews, including Override and Discrepancies; identify medication use scenarios requiring submission of Safety First Reports. Follow the ASHP-Residency guidelines as approved by Bayhealth – Complete core learning rotations as outlined in the Residency Manual. Achieve ASHP goals and objectives. Present poster at a national conference and at a regional conference (Eastern States). Fully participate in the Education Certificate Program, attend all required lectures, and complete all work necessary to achieve certificate if applicable. All other duties as assigned within the scope and range of job responsibilities.
