Job Summary Registered Nurse (RN) – Home Health – Michigan Center, Michigan Job ID #JR253445 Employment Details Duration: Ongoing 40 hours per week Shift: 8 hours Employment Type: Staff Responsibilities Lead and collaborate as a home health registered nurse case manager to manage and deliver patient care plans. Support patients in Jackson, MI and surrounding areas. Collaborate with the care team to provide high quality patient care. Consistently meet critical deadlines for charting. Self‑manage your time and schedule. Use tablets, mobile phones and EMR software to document and communicate patient information. Qualifications RN license in the state of employment. Graduate from an approved school of practical nursing. One year of RN experience with clinical competence to deliver quality patient care. Current driver’s license and ability to travel ~20% of the day to patient locations. Demonstrated experience with tablets, mobile phones and EMR software. Successful completion of a drug screen, including medical and recreational marijuana, is required prior to employment. Compensation & Benefits Compensation – Includes patient care visits, non‑visit activities, paid days off, shift premium pay, and a $10,000 first‑year success bonus. Low end of the range reflects average annual compensation for a fully productive caseload; high end reflects exceeding productivity.
