RN Case Manager: Care Coordination & Discharge Planning

RN.com is seeking an RN Case Manager based in Florida, New York. This position involves care coordination, discharge planning, and patient management across a 13-week assignment. The ideal candidate will have a FL/compact RN license and 2+ years of acute care hospital experience. The role requires weekday shifts from 8:00 am to 4:30 pm and every other weekend.
