Physical Therapist PT PRNs Needed at this facility.
Cedar Hill Nursing and Rehab Center located in Jacksonville, Florida 32244. 120 bed Rehab. Experienced DON with positive attitude.

We are looking for a committed and patient-focused Physical Therapist. Great opportunity to join a progressive therapy team. We place a high importance on attracting dynamic healthcare professionals who share a philosophy of quality patient/clinical care in a welcoming, respectful, and cohesive team environment. This person will develop, recover or maintain the daily living and work skills of people with physical, mental or developmental conditions. You will offer effective and client-centered services that will enable people to confidently and independently carry out everyday tasks or activities.
Physical Therapist Responsibilities:
Evaluate, plan and administer treatment to residents and patients with temporary or long term disabilities to relieve pain, restore or improve function, and promote healing
Assure all treatment is delivered in accordance with an established plan of care
Provide clinical support and instruct patients, families, and caregivers
Monitor patient response to treatment intervention
Complete required forms and documentation in accordance with company
Must be able to pass level 2 fingerprinting with an Eligible status.
