Physical Therapist PT

Job Summary:

The Physical Therapist in Home Health provides skilled physical therapy services primarily in the Greater Sacramento area, focusing on patient assessments, treatment planning, and therapy delivery in the home setting. The role includes supervising Physical Therapy Assistants, prioritizing start of care visits, and maintaining timely clinical documentation and communication with the care team. A valid California PT license and at least one year of home health experience are required. The position demands strong organizational skills and responsiveness during business hours to ensure quality patient care and compliance with documentation standards. Responsibilities:

• Prioritize and conduct start of care visits promptly.
• Supervise Physical Therapy Assistants effectively.
• Complete clinical notes and documentation within 48 hours of each visit.
• Report start of care visits by the next day as mandatory.
• Maintain timely communication with supervisors, intake teams, and office staff.
• Manage caseloads and visit mixes to average 8 hours per day and 40 hours per week.
• Ensure compliance with documentation systems and protocols.
• Coordinate and document therapy sessions according to point-based productivity requirements.

Required Skills & Certifications:

• Active California Physical Therapist (PT) license.
• Minimum of one year experience in Home Health physical therapy.
• Strong clinical assessment and treatment planning skills.
• Proficiency in clinical documentation and electronic medical record systems.
• Ability to supervise Physical Therapy Assistants.
• Reliable transportation and valid driver's license for travel within the territory.

Preferred Skills & Certifications:

• Experience with Kinnser documentation system or similar home health software.
• Familiarity with home health regulatory requirements and OASIS documentation.
• Strong communication and interpersonal skills for interdisciplinary collaboration.

Special Considerations:

• Mandatory next-day reporting of start of care visits.
• Responsiveness required during normal business hours.
• Overtime must be pre-approved by Clinical Supervisor.
• Travel within Greater Sacramento area with possible territory expansion.
• Use of company-issued equipment including iPad and Verizon One Talk phone system.
• Mileage, parking, and toll reimbursements provided.

Scheduling:

• Full-time, 40 hours per week.
• Caseload structured to average 8 hours per day.
• Overtime available but requires prior approval.
• No on-call, weekend, or holiday shifts required.
