Rehab Without Walls Neuro Rehabilitation Licensed Clinical Social Worker (LCSW) – Outpatient Neuro Rehab Overview We are seeking an LCSW to provide outpatient neuro rehabilitation therapy for individuals recovering from neurological conditions such as stroke, traumatic brain injury, Parkinson’s disease, and other complex diagnoses. The role focuses on psychosocial support, emotional adjustment, and functional recovery in collaboration with an interdisciplinary team including PT, OT, and SLP. What you'll do Provide outpatient LCSW neuro rehab therapy services Offer psychosocial support for stroke, TBI, and neurological recovery Conduct family and caregiver counseling and education Coordinate care within the neuro rehab clinic team Support emotional adjustment and community reintegration What makes this different Focused outpatient neuro rehab caseload Flexible scheduling options High clinical autonomy Collaborative PT/OT/SLP interdisciplinary team Meaningful 1:1 patient relationships Who we are looking for Experienced LCSW passionate about counseling services that support physical, mental, cognitive, and emotional adjustment through recovery Provides therapeutic interventions and resources that align with patient goals for personal, career, and independent living outcomes Values professional autonomy, teamwork, communication, and collaboration to maximize patient recovery Responsibilities Perform comprehensive evaluations of patients and families' social, emotional, and support needs Set measurable objectives in collaboration with the rehab team Assist physicians and other team members in understanding social and emotional factors related to patient health problems Instruct and counsel patients and families in coping with emotional responses to illnesses Participate in care coordination activities and act as a resource to other team members in identifying and resolving patient needs Assist patients and families in accessing community resources to achieve measurable objectives Qualifications Master's Degree in Clinical Social Work (LCSW) Current Licensed Clinical Social Worker licensure in the state of practice At least one year of experience in a rehabilitation setting with trauma, spinal cord injury, or acute diagnoses CPR certification required Physical ability to lift up to 50 pounds and perform patient transfers involving up to 100 pounds following safety procedures Ability to perform fine motor tasks, maintain visual acuity, and walk/stand for extended periods Comfortable with pushing, pulling, bending, stooping, reaching, kneeling, and positioning frequently Salary Range USD $37.64 / Hour #J-18808-Ljbffr Rehab Without Walls® Neuro Rehabilitation
