Job Title: Case Manager Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: 1 year in Inpatient/Acute Care, Case Management Required: Must have Epic experience License: Current nursing license Certifications: BLS Must-Have: Experience in Care Coordination Experience in Discharge Planning Inpatient Case Management experience Description: Perm candidates from other facilities are eligible for a travel contract if they have been separated for at least 3 months. Must have prior hospital experience. Travelers may be floated to other campuses if census is low. Minimum weekend coverage of 2 weekend days per month is required. Typically, this means working 1 Saturday per month and 1 Sunday per month, but not on the same weekend. This position involves care coordination for neuro and cardiac patients. It is a comprehensive stroke center. References are required for compliance. Job involves interacting with various interdisciplinary support teams. Clinicians should be comfortable with needs assessment and order DME.
