\n \nPay Range: $29/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
\n \nKey Responsibilities:
\n \n- \n
- Connect members to known community and care resources to facilitate their care management journey. \n
- Support the coordination of community outreach resources and promote awareness of available services. \n
- Assist members with inquiries regarding community and care resources. \n
- Document and maintain community resources to comply with health plan standards and policies. \n
- Collaborate with clinical teams, providing outreach, community education, and member support. \n
- Conduct non-clinical health assessments to refer members to appropriate services. \n
- Perform telephonic and in-person outreach to engage hard-to-reach individuals and families. \n
- Visit homes and community organizations as needed. \n
- Understand and address Social Determinants of Health (SDOH) barriers. \n
- Comply with all policies and standards while performing other assigned duties. \n
Key Requirements and Technology Experience:

- High School diploma or GED. \n
- 1 – 2 years of related experience. \n
