Case Manager LVN Days
Join our team in Los Angeles, CA, as a Case Manager LVN, focusing on patient care coordination and discharge planning.
Position Details:
- Contract: 13 weeks, 40 hours/week, 9am-5:30pm, includes alternating weekends
- Floating/On Call: None
- Time Off: Up to 4 days
Qualifications:
- Licenses/Certifications: Active CA LVN License, BLS, LA County Fire Card
- Experience: 1 year in case management, 2 years as LVN
- Skills: Strong communication, discharge planning, case coordination
- Preferred: Travel experience
Additional Information:
- Orientation: 5-day training
- COVID-19 Policy: No vaccination required
- Uniform: Royal Blue/Navy Blue scrubs
- Parking: On-site
- EMR: Epic
