Must-Have:
- Experience in inpatient settings preferred
- Knowledge of discharge planning, care coordination, and care management
- Driver's license required

Description:
This position requires a provisional license in the appropriate state prior to starting.
Candidates must not be employed full-time, part-time, or in a PRN capacity with any associated facilities.
Fingernail policy prohibits artificial nails in all departments providing patient care.
Refusal to comply may lead to contract termination.
The position covers inpatient environments with an emphasis on discharge planning.
A maximum caseload of 40 patients or consults may be assigned per day.
Experience with EMR systems, specifically Epic, is required.
BLS Certification is necessary.
This role may involve rotating weekend shifts.
Business casual attire is required for all staff.
Candidates should be prepared for onsite orientation as well as online training.
