Youth & Family Prevention Specialist - On-Call
Together for Youth
Together for Youth is seeking a dedicated Family Specialist to coordinate intensive case management and trauma-informed, family-centered treatment planning for at-risk youth across our community programs. You will assess family strengths, implement discharge plans, conduct home visits at least twice monthly, collaborate with schools and agencies, handle on-call duties, travel as required, and ensure documentation, confidentiality, and equitable access to services for diverse families. #J-18808-Ljbffr Together for Youth
