Job Summary:

Responsibilities:

• Assist patients and families with assessing and addressing social, emotional, and psychological needs

• Develop and implement individualized care plans to promote patient well-being and independence

• Collaborate with interdisciplinary teams, including social workers, counselors, and healthcare professionals to provide comprehensive care

• Provide emotional support and counseling to patients and families, as needed

• Facilitate patient education and discharge planning to ensure a smooth transition to the next level of care

• Utilize problem-solving skills to resolve complex social and emotional issues affecting patients and families

• Maintain accurate and up-to-date records and documentation of patient care

Work Environment:

• Work in a fast-paced, dynamic environment with a multidisciplinary team of healthcare professionals

• Assist patients and families in our short-stay and long-term care units, as well as in our therapy and rehab programs

• Participate in ongoing training and education to stay current with best practices in social work and patient care

Requirements:

• High school diploma or equivalent required; associate's or bachelor's degree in social work or related field preferred

• 0-1 year of experience in social work or a related field preferred

• PART_TIME schedule, with flexibility to work varied shifts, including evenings and weekends
