Licensed Master Social Worker

Duration: 13-Week Contract (Potential Extension) Start Date: April 13, 2026 Job Summary We are seeking qualified Social Workers to provide professional social work services supporting pediatric and adolescent patients and their families in a home care setting. The Social Worker will assess patient needs, coordinate care plans, and collaborate with healthcare providers, schools, and community resources to support positive patient outcomes and continuity of care. This position involves working with interdisciplinary teams to address social, environmental, and lifestyle factors affecting patient well‑being while ensuring compliance with healthcare regulations and professional standards. Key Responsibilities Provide social work services to pediatric and adolescent patients in a home care environment Conduct psychosocial assessments and develop individualized care plans Coordinate care with healthcare providers, school personnel, and interdisciplinary teams Support discharge planning and ensure continuity of care Identify and connect families with appropriate community resources and support services Maintain accurate documentation in accordance with healthcare regulations and organizational policies Communicate effectively with patients, families, caregivers, and care teams Participate in case reviews and care planning meetings Required Qualifications Master’s Degree in Social Work (MSW) from an accredited program Active Michigan license: LMSW or LLMSSW Few years of professional case management experience Few years of recent acute inpatient case management experience Demonstrated experience with discharge planning and care coordination Knowledge of pediatric and adolescent care practices Valid Michigan driver’s license and access to an insured vehicle for work‑related travel Preferred Qualifications Experience working with pediatric home health or community health programs Experience collaborating with schools or educational systems Familiarity with community resource networks and social service agencies Work Environment & Physical Requirements Ability to travel locally to patient homes and community locations Ability to use standard office and healthcare documentation systems Reasonable accommodations may be provided to enable qualified individuals with disabilities to perform essential job functions.
