Case Management Assistant

Job Summary And Qualifications The Case Management Assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative clerical support in Case Management functions. Coordinate with RN case manager and social worker for support needed. Communicate updates on progress throughout the day. Set up transportation via the Transfer Center and print discharge packets for post-acute care transfers. Use web-based referral system to manage post-acute care referrals. Prioritize workload to ensure a timely contact and intervention with PAC providers. Monitor patients with post-acute care referrals and intervene to expedite discharge. Communicate with patient and have the Patient Choice letter signed. Deliver the Import Medicare Message letter to the patient per facility policy. Qualifications Associate Degree preferred. Medical knowledge and terminology is required. Minimum of one year experience in a health care setting is also required, C.N.A license preferred. Possess strong organizational skills, comfortable in a fast paced environment, can multi-task along with excellent communication skills to communicate with patients, families and skilled nursing facilities. Possess the ability to work Monday - Friday, 8a - 4:30p.
