Job Summary:

NuChoice Health Partners, a home care agency serving the Northwest of Pennsylvania, is seeking a dedicated Personal Care Assistant to join our team. As a Caregiver, you will provide essential support to patients in their own homes, promoting independence, comfort, and overall well-being.

Responsibilities:

• Assist patients with daily living activities such as bathing, grooming, and dressing
• Prepare and cook meals, promoting proper nutrition and hydration
• Provide medication reminders and monitor medication adherence
• Offer companionship and emotional support to patients, enhancing their mental health and social well-being
• Help patients with mobility exercises and maintain a safe and clean environment
• Collaborate with healthcare professionals to ensure the well-being of patients
• Document tasks completed and time in and out, maintaining accurate records

Needed Skills:

• Basic Housekeeping
• Cooking
• Bathing Patients
• Strong communication and interpersonal skills
• Compassionate and empathetic attitude towards patient care
• Willingness to learn and adapt to different healthcare settings

Work Environment:

As a Caregiver, you will work in patients' homes, providing one-on-one care and support. This role requires a strong sense of independence, flexibility, and the ability to work in a fast-paced environment.
