Job Opportunity: Physical Therapist

Our organization is in search of a committed and empathetic Physical Therapist to become a vital part of our Healthcare & Medical Services team. In this position, you will deliver exceptional, evidence-based rehabilitation services to patients across various care settings, including skilled nursing facilities, outpatient clinics, and home health environments, depending on the needs of the organization.

As a Physical Therapist, your responsibilities will include:

• Conducting thorough evaluations and assessments of patients.
• Creating personalized care plans tailored to individual patient needs.
• Implementing therapeutic interventions that align with best clinical practices and regulatory guidelines.
• Educating patients, families, and caregivers on home exercise routines, safety measures, and wellness strategies to promote long-term health.
• Regularly reassessing patient progress and adjusting treatment plans as necessary.
• Accurately documenting all services provided in our electronic medical record system.

You will collaborate closely with a multidisciplinary care team, which includes physicians, nurses, occupational and speech therapists, social workers, and other professionals. Your role will involve coordinating patient care, participating in care plan meetings, and contributing to discharge planning. Advocacy for patient needs and maintaining clear, professional communication will be essential, as will upholding our commitment to dignity, respect, and patient-centered care.
