Part Time / Weekend Physical Therapist Case Manager: Home Health

Part‑Time/Weekend Physical Therapist Case Manager – Home Health Join 21st Century Home Health Services (21HHS), the largest home‑health agency in San Francisco, as a Part‑Time Physical Therapist Case Manager. This role requires field visits to patients’ homes to assess, treat, and coordinate care. Pay : $100–$150 per visit. Minimum 15 visits per week on a 3‑day workweek. Mileage reimbursed at IRS standard rate. Additional $20 for missed visits, $35/hour for non‑visit work. What you’ll do Perform in‑home evaluations and develop individualized care plans. Coordinate with PTAs to ensure plan adherence and support patient independence. Assist patients with mobility and daily activities. Communicate progress to physicians and report to MDs. Strong clinical decision‑making, empathy, organizational, and leadership skills. Experience with interdisciplinary teams and case‑management fundamentals (highly desired). Self‑directed, critical thinker, outstanding interpersonal skills. Employment Details Entry‑level, part‑time, Health Care Provider. Salary range: $111,168 – $172,310 annually.
