Job Summary

We are hiring a Travel Physical Therapist (PT) to provide skilled therapy services in a skilled nursing and rehabilitation setting. Youll evaluate patients, develop personalized treatment plans, and help improve mobility, function, and quality of life while working with a collaborative healthcare team.

Responsibilities
Evaluate and treat patients.
Develop and update individualized treatment plans.
Document patient progress accurately.
Educate patients and families.
Supervise PTAs and Rehab Technicians.
Collaborate with the interdisciplinary care team.
Maintain compliance with facility and regulatory standards.
Qualifications
Graduate of an APTA-accredited Physical Therapy Program (DPT/MSPT/BSPT).
Active New Hampshire PT License or ability to obtain.
Strong communication, documentation, and clinical skills.
SNF or rehabilitation experience preferred.
