• Key Requirements Summary: 1. Experience: 1 full year of acute care hospital experience required (hard stop); 2 years listed in system; will not accept candidates with experience specialized only in Behavioral Health or ED 2. Cases: Caseload of 20-23 patients; unit-based discharge planning; adults and geriatrics; initial assessment within 24 hours of admission; Social Workers function as Case Managers/Discharge Planners 3. Certifications: BLS required; LCSW or MSW required; must be AHA or Americ
