Job Description:

PRN Days - Up to 4 Days to be Worked per week

5 Hours per Shift

Start Time of shift - varies between 8a - 2p

End Time of shift - varies between 1p - 8p

Job Summary

Responsible for coordinating and assessing all patients referred to intensive outpatient programming.
Coordinates the approval and level of care determination in cooperation with the psychiatristpatient/family and insurance verification.
Coordinates and facilitates outside referrals and placement if needed

• Participates in the formulation of a treatment planimplementation and evaluation
• Implement therapeutic evidenced based group therapy
  and psychoeducational classes to meet the needs of the patients
• Provide brief therapeutic counseling
• Assist patients with paperwork needed for FMLA and
  Short-term Disability in order to be away from work while in treatment
• Develops and maintains professional relationships with
  treatment team members hospital and medical staff and communitypartners

Minimum Education Experience Training and Licensures Required

• Masters Degree required
• Active Kentucky License: LCSW LPCC or LMFT
• 2 years of job-related experience required
• Experience working in a hospital setting; knowledge of substance use disorder treatment; experience in group therapy preferred
