PRN Physical Therapist (PT) - Woodland, CA Cottonwood Healthcare Center is seeking a Physical Therapist to join our rehab team. In this role, you will support high-quality physical therapy services across Skilled Nursing setting, helping residents regain function, independence, and confidence in daily life. Why You Will Love This Opportunity Competitive pay (typically $60–$68 per hour, depending on experience) Supportive, collaborative therapy team committed to excellent patient care Meaningful work where you can directly enhance resident quality of life Flexible scheduling options Position Summary Evaluates, plans, organizes, develops, and directs the Physical Therapy (PT) services for assigned patients/residents in accordance with current applicable laws and regulations, and as may be directed by the Director of Rehabilitation and/or Administrator, to assure that the highest degree of quality care can be maintained. Qualifications Current PT license Experience as a therapist in a nursing, hospital, or other related medical facility. Training in rehabilitative and restorative therapy practices Current CPR/BLS certifications Proven organizational and communication skills Ability to remain calm and professional in all work-related situations Responsibilities Plans, develops, organizes, implements, evaluates, and directs the physical therapy for assigned patients/residents in accordance with current laws and regulations that govern nursing care facilities Meets with administration, medical and nursing staff, as well as other related departments in planning therapy services Maintains treatment records, resident files, and progress notes as required Assesses patients’ needs; identifies and initiates appropriate physical therapy interventions Observes and evaluates treatment outcomes recommending changes as needed Performs assessments and re-assessments as needed on each assigned patient/resident Interviews resident to determine interests and types of treatments needed Participates in development and implementation of resident assessments (MDS) and care plans, including quarterly reviews Assists in transporting resident to and from physical therapy room/area Documents patient care in accordance with regulatory, licensing, payer and accrediting requirements Educates patient\'s family/nursing staff/caregivers in follow-through programs Maintains equipment and work area in a safe and clean condition We are proud to be an equal opportunity employer.
