They will get an email from the CS team with instructions on how to make an appointment. This will be a 15 Panel Drug Screen and must include Amphetamine, Cocaine Metabolites, MDMS Screen, Barbiturates, Benzodiazepines, Marijuana Metabolite, Opiates, Fentanyl Screen, Methadone, Propoxyphene Metabolite, Phencyclidine, Oxycodone/Oxymorphine, Meperidine Screen (Urine), Tramadol and Buprenorphine screen Demographic Sheet: Demographic Sheet is required within 48 hours of signing.
