Role Responsibilities

• Deliver comprehensive, continuous care to patients of all ages, from children to seniors.
• Address and manage chronic conditions, preventive health, acute issues, and mental health matters utilizing a patient-centric approach.
• Perform physical examinations, analyze diagnostic tests, and create personalized care plans.
• Partner with care coordinators, behavioral health experts, and specialists to provide holistic health services.
• Engage in quality enhancement initiatives aimed at improving patient care and operational efficiency.
• Maintain precise and current patient records using electronic medical documentation systems.
• Offer care options that include in-person visits and virtual consultations to cater to diverse patient needs.

Education Requirements

• MD or DO from an accredited medical institution.
• Completion of a Family Medicine residency program accredited by the ACGME.

Certifications

• Board Certification in Family Medicine.
• Possession of (or eligibility for) an active medical license in New York.
• BLS and ACLS certification preferred.

Skills and Experience

• Strong clinical and diagnostic abilities with a focus on preventive and proactive healthcare.
