Hospice RN Case Manager - Patient-Centered Home Care
The Care Team
The Care Team Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) to deliver skilled nursing care and end-of-life support for patients in home and facility settings. You will collaborate closely with the interdisciplinary team to coordinate care and ensure high-quality, patient-centered outcomes. Standard hours are Monday–Friday, 8:00 AM–4:30 PM in Wayne County, MI, with potential on-call duties for additional pay. #J-18808-Ljbffr The Care Team
