Weekend Baylor Rn Registered Nurse - Home Health

Empath Home Health, a member of Empath Health is seeking a compassionate Weekend Baylor RN Registered Nurse - Home Health to work with our Home Health team to deliver individualized care to patients in their homes throughout Lee county (Fort Myers, Bonita Springs, Estero, and surrounding areas).

Locations: Home health visits in Lee county (including Fort Myers, Bonita Springs, Estero, and surrounding areas).

Schedule: This is a Weekend Baylor role requiring full weekends. You decide which schedule below fits your life!

• 4 days (Friday-Monday, Thursday-Sunday, OR Saturday-Tuesday)
• 3 days (Friday-Sunday OR Saturday-Monday)

What you'll do

• Performs initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
• Initiates the plan of care and revise as necessary.
• Provides nursing services and/or treatments indicated on plan of care.
• Counsels and educates the patient and family on disease process, self-care techniques and prevention strategies.

What You'll Need

• Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
• BSN or MSN preferred.
• Minimum one year experience providing direct patient care; previous experience as a home health RN preferred.
• Home Health RN experience required
