Key Responsibilities

• Provide occupational therapy treatment to patients under the supervision of a licensed Occupational Therapist
• Implement individualized treatment plans to support patient rehabilitation and functional independence
• Document patient progress accurately and in accordance with facility and regulatory standards
• Collaborate with the interdisciplinary care team to ensure coordinated patient care
• Educate patients and caregivers on therapeutic techniques and home exercise programs

Required Qualifications

• Current COTA certification through NBCOT
• Active and unrestricted state licensure in Illinois
• Experience delivering occupational therapy services in a clinical or rehabilitation setting
• Strong documentation and communication skills

Preferred Experience / Skills

• Previous experience in a skilled nursing or long-term care environment
• Familiarity with electronic medical records (EMR) systems
