Job Responsibilities:
•Evaluate, diagnose,and treatpatients with physical impairments in a SkilledNursing Facilitysetting
•Develop and implementindividualized treatment plansbased on patient needs andclinical findings
•Supervise andcollaborate with PhysicalTherapist Assistants to ensure effectivecare delivery
•Monitor patient progress and modify treatmentplans asneeded
•Educate patients and caregivers on mobility,safety, and home exercise programs
•Maintain accurate andtimely documentation in compliance with facility and regulatorystandards

Requirements
•Active state license as a Physical Therapist in California
•Current BLS/CPR certification
•Minimum of 1 year of recent PTexperience preferred, ideally in a SNF or geriatric setting
•Strong clinical reasoning and leadership skills
•Ability towork collaboratively within an interdisciplinary team
