Job Description: Job Title: LPN - Clinic Job Type: Contract Duration: 13 Weeks Shift: Days Guaranteed Hours: 36 hours/week Experience Required: Minimum 2 years License: Must have an active state LPN license Job Summary / Purpose The Licensed Practical Nurse (LPN) performs and documents patient assessments within their scope of practice. This role ensures completion of health history and admission processes, initiates appropriate interventions or referrals, and identifies discharge planning and education needs. The LPN provides patient care, assists with personal care needs, communicates patient progress, and manages workload effectively. Key Responsibilities Assist the registered nurse or licensed practitioner in identifying patient needs, care priorities, and goals based on individual factors. Observe, record, and report patient responses to treatments and contribute to care plan evaluations. Modify care plans in collaboration with the healthcare team. Perform duties in accordance with department standards, policies, and procedures. Work as part of a multidisciplinary team to deliver patient-centered care. Delegate tasks appropriately to qualified personnel. Monitor patients for any changes in condition and report promptly. Administer treatments and medications safely and document care accurately. Communicate effectively with patients, families, and healthcare team members. Report patient pain and symptoms according to protocol. Participate in patient education as directed by a licensed practitioner. Assist with discharge planning, education, and counseling for patients and families. GSK Solutions
