Home Hospice Registered Nurse

You will conduct comprehensive nursing assessments, develop and implement hospice care plans, manage complex symptoms, and collaborate with an interdisciplinary team that may include physicians, nurse practitioners, social workers, chaplains, and hospice aides. Your typical day will include driving to patient homes, performing focused and holistic assessments, administering medications and treatments, educating patients and families on comfort-focused care, and providing emotional support during a vulnerable and meaningful time in their lives. You will coordinate closely with the care team to adjust plans of care, respond to changes in condition, and ensure smooth communication among all involved in the patient's support network. Patient caseload is structured to allow for thorough visits and meaningful time at the bedside, with a strong emphasis on symptom relief, dignity, and honoring patient and family goals. You will document all care in an electronic medical record and participate in regular team meetings, case conferences, and on-call rotations as assigned. Shifts typically include daytime field hours with some flexibility to accommodate patient needs, and may include participation in evening or weekend coverage.
