Clinical Case Manager (NHC-1474722) Pflugerville, Texas Salary: USD52000 - USD62000 per year Clinical Case Manager Location: Austin, TX Organization Type: Nonprofit Supportive Housing & Community Services Organization Requirements Bachelor’s degree from an accredited college or university. Active Texas licensure such as LBSW, LMSW, LCSW, LPC-A, LMFT, LCDC, or another applicable clinical license. Minimum of 1 year of professional experience in case management, behavioral health, social services, rehabilitation, housing programs, or a related field. Valid Texas driver’s license, reliable transportation, and ability to transport clients as needed. Job Duties Manage a caseload of individuals experiencing severe and persistent mental illness, substance use disorders, chronic health conditions, and housing instability. Develop, implement, and monitor individualized service plans, treatment goals, and supportive interventions in collaboration with clients and the multidisciplinary team. Provide clinical case management, crisis intervention, symptom education, advocacy, and support using Housing First, harm reduction, and trauma-informed care principles. Connect clients to community resources, including medical care, behavioral health services, substance use treatment, benefits, education, employment, and transportation. Support housing placement, housing retention, and long-term housing stability efforts. Maintain accurate and timely client documentation, case notes, reports, and database records in compliance with program requirements. Collaborate with internal staff, healthcare providers, community agencies, and other stakeholders to coordinate services and improve client outcomes. Additional Information Schedule: Monday-Friday, 8:00 AM-5:00 PM (40 hours per week) Employment Type: Contract-to-Hire Compensation: $52,091-$62,278 annually, depending on experience Preferred Qualifications: Master’s degree in Social Work, Counseling, Psychology, Public Health, or a related human services field; experience working with individuals experiencing homelessness or mental health challenges; bilingual proficiency preferred.
