Responsibilities:

• Clinical Leadership: Provide leadership and guidance to a team of nurses, therapists, and home health aides to ensure effective patient care and outcomes.

• Patient Care: Collaborate with patients, families, and healthcare providers to develop and implement personalized care plans that meet the unique needs of each patient.

• Staff Supervision: Supervise, mentor, and evaluate the performance of clinical staff to ensure compliance with clinical standards and organizational policies.

• Quality Improvement: Participate in quality improvement initiatives to enhance patient safety, reduce readmissions, and improve overall clinical outcomes.

• Regulatory Compliance: Ensure compliance with regulatory standards, accrediting organization requirements, and organizational policies and procedures.

Requirements:

• Education: Bachelor's degree in Nursing (BSN) or higher

• Experience: Minimum 2 years of experience in home health care or a related field; direct patient care experience as a Registered Nurse (RN)

• Certifications: Current RN license; certification in a relevant field (e.g., CRRN, CCM, CPN)
