Case Management Registered Nurse

Location: Eureka, California

Assignment Duration: This is a 13-week contract position.

Schedule: Day Shift: 5x8-Hour (08:00 - 16:30). Required to work every other weekend.

Job Description: We are seeking an experienced Case Management Registered Nurse with acute care experience to join our team. In this role, you will provide comprehensive care coordination for patients, ensuring all needs are met through effective case management practices. You will work closely with interdisciplinary teams to assess, plan, implement, and evaluate care plans while adhering to applicable regulations and guidelines.

Responsibilities:

• Conduct initial assessments and develop individualized care plans for patients.
• Coordinate patient care across multiple disciplines and settings.
• Manage cases involving appeals and denials, ensuring compliance with CMS guidelines.
• Oversee discharge planning, facilitating smooth transitions for patients.
• Assist with prior authorizations and benefit eligibility determinations.

Required Experience / Certifications / Licensure:

• Must have a minimum of 2 years of acute care experience.
• Current nursing license in California.
• Experience with HMOs, IPAs, and advance directives is highly preferred.
• Familiarity with the Epic charting system.
