Responsibilities Provides a comprehensive assessment of a patient's psychosocial needs and develops treatment plans and reviews based on findings and department policy. Provides behavioral health treatment and education, including individual and/or group assessment or therapy, and shares and interprets information with staff, families, and approved agencies. Completes all documentation and reports in a timely fashion in accordance with department and hospital policy. Provides services in accordance with licensure regulations. Utilizes data to determine appropriate evidence‑based treatment options for specific populations. Provides comprehensive case management services and assessment as warranted. Assesses social determinants of health (SDOH) and offers information, education, or referrals as appropriate. Collaborates with patients, families, and support persons to develop comprehensive discharge plans. Reassesses treatment plans, evaluates progress, and revises plans when clinically indicated. Maintains protected patient health information as required by law and policy. Assists team members in responding to emergencies. Has working knowledge of community agencies and resources and refers patients when appropriate. Assists with interview and orientation of new staff members. Supervises social work/counseling students during field placement. Identifies and meets specific needs of the patient population served, including age, culture, language, and accessibility considerations. Exerts a minimum of 5 lbs. for lifting, pushing, or pulling, and stands a minimum of 4 hours per day. Adheres to the standards identified in the Medical Center’s organizational competencies. Receives proper supervision as per licensing board requirements. Performs additional duties as assigned and as specified during the interview process and at time of hire. Qualifications Master’s degree in Social Work (MSW). Minimum of 2 years of post‑licensure work experience. Ability to perform psychosocial assessments and develop treatment plans. Knowledge of crisis intervention theory and disease management. Ability to work as part of a team and build collaborative relationships. Strong follow‑through and attention to detail. Excellent written and verbal communication skills. Proficient computer skills (Microsoft Office and/or Google Suite). Preferred Ability to provide guidance and mentorship to other team members. Licenses and Certifications NJ Licensed Clinical Social Worker. Minimum hourly rate is $43.16 with pay based on experience, education, and other factors.
