Vighter is recruiting Wound Care, Ostomy and Continence Registered Nurses (RN) to provide exceptional patient care at Brooke Army Medical Center in San Antonio, TX. The RN provides the full range of nursing services within the scope of practice related to this specialty and ensures that all JCO standards for care are met during performance of patient care. RNs provide complex and comprehensive professional nursing services within the professional scope of practice for a variety of diagnoses for patients of all ages.

The RN performs comprehensive assessments of acute and chronic wounds, including pressure injuries, diabetic wounds, surgical wounds, and skin breakdown. They develop individualized wound-care treatment plans based on wound type, stage, drainage, tissue condition, and patient needs. They perform and manage wound treatments, including dressing changes, wound cleansing, packing, and negative-pressure wound therapy when indicated. They assess patients for pressure-injury risk and implement prevention strategies. They evaluate ostomies and peristomal skin for complications such as irritation, leakage, infection, or retraction. They select and apply appropriate ostomy appliances, barriers, pouches, and accessories. They educate patients and caregivers on ostomy care, pouch changes, skin care, nutrition, and lifestyle management. They provide continence assessment and management for urinary and fecal incontinence. They recommend appropriate skin-care products and interventions to prevent moisture-associated skin damage. They monitor healing progress and modify treatment plans as needed. They collaborate with physicians, surgeons, dietitians, physical therapists, case managers, and bedside nurses. They make recommendations regarding specialty beds, pressure-relieving devices, support surfaces, and other wound-care equipment. They provide consultation and education to nursing staff regarding wound, ostomy, and continence management. They document assessments, treatments, patient responses, and progress accurately in the medical record. They participate in multidisciplinary rounds and develop discharge plans for ongoing wound or ostomy care. They coordinate follow-up care and referrals to outpatient wound or ostomy services when needed. They promote evidence-based practices, infection prevention, and patient safety. They attend and participate in a variety of meetings and task force groups to improve quality and outcomes, communicate issues, obtain approvals, resolve problems, and maintain a specific level of knowledge of new developments, new task efforts, and new policy requirements. They demonstrate functional proficiency and ability to document patient-centered health care in the required electronic health record or electronic medical record, in the required format, in a timely manner, and in compliance with contract policy and guidance. They have occasional schedule flexibility with an end state of 100% staffing and 100% service to the patients. They have an unshakeable adherence to quality and continuous process improvement. They have an apolitical, patient-centered focus that drives to enhanced care and patient satisfaction. They have knowledge of regulations (HIPAA/Privacy Act) regarding the confidentiality of patient medical records and information as well as Personally Identifiable Information (PII).

The RN must have exceptional customer service skills. They shall be technically proficient in the skills necessary to fulfill the job requirements, including ability to speak, understand, read and write English fluently. They must be able to work in a fast-paced changing environment with limited supervision. They must have excellent organizational, scheduling and prioritization skills. They must have an Associates or Bachelor of Science in Nursing from an accredited university. They must have a minimum of 2 (two) years of experience as an RN in the last 4 (four) years working as a wound ostomy and continence nurse (WOC). They must have Wound Ostomy Continence Nurse Credentialing Board (WOCNCB). They must have Wound Ostomy Continence Nurse Education Program (WOCNEP). They must have graduated from an accredited WOCN education program. They must have and maintain certification in Basic Life Support or Advanced Cardiac Life Support (ACLS) through the American Heart Association. They must have strong communication and interpersonal skills. They must have a commitment to patient-centered care and evidence-based medicine. They must be able to work independently while being a collaborative team player.

The expected duty hours for this position are 8 hours per day and 5 days per week. However, this key position supports 24/7/365 operations which may require work to be conducted in the day, at night, on weekends, and during holidays. This position has availabilities to work day, evening and night shifts. Availability during weekends and holidays is a possibility. Advanced notice will be provided for scheduling purposes.

The work environment is located in San Antonio, TX. It includes a supportive team of healthcare professionals and administrative staff. As required to satisfy the contractual obligations, commuting is non-compensatory. All official travel is government funded.

The physical requirements include the ability to stand or sit for more than 8 hours per day. They must be willing to work overtime if necessary. They must be able to use an elevator or walk up and down stairs. They must be able to lift up to 40 pounds and carry up to 10 feet. They must be able to routinely grasp or handle objects, use finger dexterity, bend elbows/knees and reach above and below shoulders. They must be able to read and interpret handwritten and typewritten print. They must be able to communicate by voice and detect sound by ear. They must be able to perform duties in an indoor or outdoor environment. They must be able to pass a criminal background investigation. They must undergo Urine Drug Screening (UDS). They must be a Citizen of the United States or legally authorized to work in the United States of America.
