Job Details Location: New Hartford, NY 13413 Position Type: Full Time Education Level: 4 Year Degree Salary Range: $75,000 - $85,000 Shift: Day (Monday - Friday 8a – 4p, holidays and weekends off) Department: Neurological Disease Unit Supervisor: NeuroCare Director, Director of Nursing Sign-On Bonus: $10,000 Responsibilities Provide clinical nursing oversight of the unit, 32 long‑term NeuroCare beds, 24‑hour period in conjunction with Nursing Supervisors. Work with Neuro Unit direct care staff and Program Director to implement care plan changes as needed relative to each resident’s disease process and subsequent declines. Complete quarterly, annual, and significant change nursing MDS assessments for all residents on the Neuro Care unit as scheduled by the MDS coordinator. Monitor Neuro Unit staff for proper implementation of departmental nursing policies and procedures and identify areas in need of training, communicating such needs and making appropriate employee referrals to Education and Training. Work with Neuro Program Director to complete in‑person assessments of potential Neuro Unit residents, as designated by the Director. Provide input for completion of nursing staff probationary performance evaluations. Regularly inspect the unit to ensure compliance with federal, state, and local standards. Assist with contacting ancillary departments when issues arise related to provision of services either directly or environment requiring attention. Provide clinical updates to resident families inclusive of falls, injuries, medication changes, hospitalizations, clinical declines, acute changes of condition. Attend weekly care plan meetings for residents. Ensure completion of Fall Risk Assessments and Braden Scales prior to review. Complete trainings and ensure compliance with all infection control standards and procedures. Provide facility supervisory shift coverage as deemed necessary by facility needs/D.O.N. Actively participate in Quality Assurance Performance Improvement Committee, Safety Committee, Leadership Meetings, Morning Report, Skin Care Meetings. Perform duties according to the highest ethical and moral standards in adherence with the facility’s corporate compliance plan and applicable governing laws. Adhere to the facility’s confidentiality statement in accessing and safeguarding protected health information (PHI) of residents and employees, as necessary to perform job duties. Perform any other duties throughout facility to meet needs as assigned by the D.O.N. Qualifications Graduate of an approved/accredited School of Nursing and currently licensed as a Registered Professional Nurse in New York State. Bachelor’s degree in nursing preferred. Previous leadership experience with at least two years of supervisory experience in the nursing field; previous experience with NeuroCare and/or Neurological Disease preferred. Knowledge of Federal and State Regulations. Ability to organize, plan and implement nursing service policies and procedures. Displays an active interest in pursuing continuing education to keep knowledge and skills current. Knowledge of completion Minimum Data Set (MDS) preferred; demonstrates competency by completing an annual job‑specific skills checklist.
