Manager of Case Management

A leading Houston healthcare system is seeking an experienced Manager of Case Management to oversee daily case management operations across the trauma continuum. Reporting to the Director, this leadership role focuses on care coordination, utilization management, discharge planning, and quality outcomes.

Responsibilities

• Manage daily case management operations and staff.
• Oversee care facilitation, utilization review, and discharge planning.
• Collaborate with interdisciplinary teams to ensure quality patient outcomes.
• Monitor performance metrics and support quality improvement initiatives.
• Assist with resource management and departmental goals.

Qualifications

• Active CCM (Certified Case Manager) certification required.
• Current Texas RN or LMSW license required; LCSW preferred.
• 5+ years of case management, utilization management, or discharge planning experience.
• 3+ years of hospital-based nursing or acute care social work experience.
• 3+ years of healthcare leadership or supervisory experience.
