Position Summary The Certified Nurse Aide (CNA) - Rehab assists nursing personnel in providing direct and indirect patient care to rehabilitation patients under the supervision of a Registered Nurse. The CNA supports activities of daily living, promotes patient comfort and safety, and contributes to the overall rehabilitation process while maintaining compassionate, patient-centered care. Essential Functions Assists patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, feeding, and mobility. Obtains and records vital signs, weights, intake and output, and other patient care data as directed. Assists with ambulation, transfers, positioning, and range-of-motion exercises. Observes and reports changes in patient condition, behaviors, or concerns to the licensed nurse promptly. Maintains a safe, clean, and organized patient environment. Answers call lights and responds promptly to patient needs. Assists with patient admissions, discharges, and transfers. Provides emotional support and encouragement to patients during their rehabilitation journey. Assists with transporting patients to therapy sessions, diagnostic procedures, and other departments as needed. Ensures compliance with infection prevention, patient safety, and hospital policies and procedures. Documents care provided accurately and timely in accordance with hospital standards. Collaborates with nurses, therapists, physicians, and other healthcare team members to support positive patient outcomes. Required Certifications/Licensure Current Kentucky Nurse Aide Certification required. Current Basic Life Support (BLS) certification required within established timeframe. Required Skills Ability to provide compassionate, patient-centered care. Effective communication and interpersonal skills. Ability to prioritize and manage multiple tasks in a fast-paced environment. Strong observation and documentation skills. Basic computer and electronic medical record proficiency. Ability to work effectively as part of an interdisciplinary healthcare team. Preferred Qualifications Previous experience as a Certified Nurse Aide in an acute care hospital, rehabilitation, long-term care, or skilled nursing environment preferred. Experience caring for rehabilitation patients preferred.
