Job Type: Full-Time Pay: Weekly Pay Requirements: High School Diploma/GED; 2+ years of healthcare, scheduling, or patient services experience preferred Location: N Chesterfield, VA About the Position Caremed Inc. is hiring a Patient Care Coordinator – Home Care to coordinate patient care, caregiver schedules, home care services,EVV, and daily client scheduling operations. This fast-paced role involves patient scheduling, caregiver assignments, inbound/outbound calls, EVV, visit verification, cancellations, call-outs, missed visits, reassignments, and staffing coverage . Key Responsibilities Coordinate patient and caregiver schedules Manage open shifts, call-outs, cancellations, and missed visits Participate in a rotating on-call schedule for after-hours and weekend coverage Respond promptly to urgent staffing, scheduling, and patient care coordination needs while on call Perform EVV and visit verification Communicate with patients, caregivers, RN Supervisors, and office staff Resolve scheduling issues and patient/caregiver concerns Maintain accurate scheduling and patient information Follow home care policies, documentation, privacy, and compliance requirements Qualifications High School Diploma or equivalent 2+ years of healthcare, home care, medical office, scheduling, or patient services experience preferred Strong communication, organization, multitasking, and problem-solving skills Experience with EVV, Medicaid, patient authorizations, or Plans of Care preferred Microsoft Word, Excel, Teams, and Outlook proficiency Ability to work independently in a fast-paced healthcare environment Why Join Caremed?
