Physical Therapist Outpatient FT

Responsibilities Take the lead by prioritizing, coordinating, and delegating all physical rehabilitation care touchpoints for your patients. Interact with other members of the rehabilitation team to coordinate goals and improve quality of care. Ensure patient physical therapy care and activities are consistent with the medical plan of care. Maintain and advocate a safe environment for everyone. Monitor and engage with patients, families, and appropriate care team members for teaching and education opportunities, patient condition changes, or patient questions and concerns. Ensure continuity of care, efficient throughput, consistent communications, and an exceptional experience for every patient. Contribute to the team by assisting others without being asked. Provide guidance to new and experienced personnel and oversight for ancillary personnel. Reassess and revise goals and treatment plans as indicated by patient status, program requirements, and CMS rules and regulations. Evaluate patient progress toward expected outcomes and document progress and interventions. Maintain appropriate data and contribute to department quality management activities, recommending systematic and programmatic improvements based on data. Demonstrate skill and accuracy in performing age‑appropriate assessments of patients, documenting results of standardized assessments, and serving as a resource for program managers and directors to achieve accreditation standards. Qualifications Must‑Haves: B.S., M.S., or Doctorate in Physical Therapy. Current license to practice Physical Therapy in the state of Louisiana. American Heart Association Basic Life Support/CPR certification (can be obtained on board). Non‑Violent Crisis Intervention certification (can be obtained on board). Preferred: 1‑year experience in setting. Additional certifications in manual therapy, dry needling, vestibular rehabilitation, or women’s health. New graduates are welcome to apply.
