Responsibilities
- Provide direct, hands‑on nursing care to residents in a long‑term care setting
- Administer prescribed medications and treatments according to physician orders and facility policy
- Monitor and document vital signs, intake/output, hydration status, and changes in condition
- Perform focused head‑to‑toe assessments and promptly report acute changes to the care team
- Assist residents with activities of daily living, mobility needs, and safety precautions
- Collaborate with the interdisciplinary team to develop and adjust individualized care plans
- Maintain accurate charting, ensure proper communication during shift changes, and uphold standards of resident safety and dignity

Requirements
- Active Licensed Practical Nurse license in New Mexico
- Graduation from an accredited LPN program and current LPN credential
- Local candidates only
- Ability to work 3x12-hour day shifts (approximately 12-hour shifts scheduled across three days)
- Strong communication, documentation, and teamwork skills relevant to a skilled nursing environment
