Roles & Responsibilities
- Review and interpret electronic physician orders to confirm accurate drug choices, safe dosages, and proper administration routes
- Evaluate patient profiles for allergies, drug-drug interactions, and lab results
- Oversee or personally prepare specialized medications, including large-volume parenterals, intravenous (IV) piggybacks, total parenteral nutrition (TPN), and chemotherapy infusions
- Participate in medical rounds with physicians and nurses to recommend evidence-based therapies and assist in medical emergencies
- Conduct medication reconciliation during a patient's admission and provide comprehensive drug counseling to patients upon hospital discharge
- Manage, review, and delegate tasks to pharmacy technicians, interns, and students to maintain accurate inventory and cart distributions
Qualifications
- 2 years of inpatient pharmacist experience
- Michigan state license
- Ability to work in the Anticoagulation Clinic and perform Oncology compounding
Education
- Pharmacy degree from an accredited institution
