Registered Nurse Nurse Care Coordinator

Job Summary:

The Nurse Care Coordinator provides comprehensive, patient-centered nursing care in a fast-paced clinical environment, ensuring optimal health outcomes through assessment, planning, implementation, and evaluation of patient care. This role requires strong clinical judgment, interdisciplinary collaboration, and adherence to safety and regulatory standards.

Responsibilities:

• 
  Perform thorough patient assessments and develop individualized care plans.
  
  
• 
  Administer medications and treatments accurately according to provider orders and policy.
  
  
• 
  Recognize significant changes in patient condition and initiate appropriate interventions or escalate care.
  
  
• 
  Educate patients and families on care procedures, medications, and discharge instructions.
  
  
• 
  Document assessments, interventions, and patient responses in the electronic health record accurately and promptly.
  
  
• 
  Collaborate with interdisciplinary team members to coordinate ongoing care, referrals, and follow-up services.
  
  

Qualifications:

• 
  Possess an active, unrestricted RN license in the State of Alaska.
  
  
• 
  Hold an associates degree in Nursing from an accredited program.
  
  
• 
  Maintain current BLS, ACLS, and PALS certifications; additional area-specific certifications obtained within six months of hire.
  
  
• 
  Demonstrate strong clinical judgment, critical thinking, and effective communication skills.
  
  
• 
  Show proficiency with electronic health records and clinical systems.
  
  
• 
  Be able to lift up to 50 pounds, work extended hours, and operate safely in a clinical setting.
